What is Domiciliary Care?
Domiciliary care refers to help or treatment someone receives in their own home. This can range from personal care and companionship to more specialist care, such as complex health care, dementia and palliative care.
Everyone’s circumstances are different and can change throughout the course of a care plan. For example, someone discharged from the hospital may need substantial support, which will gradually ease off as they recover.
What we do:
- Assistance with bathing, washing, dressing up or undressing.
- Food and meal preparation.
- Help to get out of or into bed.
- Assistance with shopping, prescription collection and overseeing medication.
- Sitting-in and companionship.
- Accompaniment to appointment.
- Hospital discharge and rehabilitation support.
- General household tasks and laundry.
- Feeding and taking care of your pet, including dog walking.
Visits To Suit You
Our flexible approach to personal care means we can provide as many daily or weekly visits as you need, with the option to introduce additional steps such long day/night as well as live-in care and specialist care if needed. We will ensure you get the help you need whenever you need it.
Live-in care means there is always someone to attend to your needs every day or night for 24 hours a day. Our specifically selected caregivers provide professional care that meets your personal needs.
We provide reassurance so that you and your loved one feel safe and secure as you retain your sense of independence in your own home.
Circumstances can change suddenly so that home care is required as quickly as possibleWe are always on hand to help. Our team of qualified professionals are ready for an urgent need.
Complex care is a package of care which is arranged and solely funded by NHS Continuing Healthcare (NHS CHC) for adults aged 18 and over. An assessment is carried out by a Clinical Commissioning Groups (CCG) to determine a ‘primary need’ and eligibility for funding.
If one is eligible, we provide support in your own home through either the option of direct funding from NHS CHC or a Personal Health Budget (PHB). We work with NHS South West London and Surrey Heartlands Integrated Care Boards (ICB)
Looking after a loved one can be very demanding. Often family members put the other person first, neglecting themselves in the process. At Millfield Healthcare, we provide a range of respite packages, including overnight and live-in care. This will allow you, as a dedicated caregiver, time to rest and recuperate without feeling guilty or worried that your loved one is on their own.
Our professional, dedicated, and carefully vetted caregivers provide everything from overseeing medication to mobility support and continence care.
This way, your loved one can continue to live at home whilst receiving personalised care designed to enhance their independence and safety and maximise their quality of life. At the same time, this will allow you respite for your well-being, and you will find time for other essential duties.
Hospital Discharge
If your loved one is getting discharged from the hospital and wants someone to be there for them, we are here to help. From navigating the discharge to arriving home, we are here to make things as stress-free as possible. We work with hospitals on the Discharge to Assess (D2A) and Non-Weight Bearing (NWB) program. We provide care for your as the hospital continues to assess your recovery progress over a period of two to six weeks.
Companionship
Sometimes the little things in life matter, like spending time with someone and taking the time to talk to them. Our goal is for your loved one to be given a new lease on life and never feel alone.
Palliative care is support and treatment for people with life-limiting or terminal illnesses. This can include assisting with washing, getting dressed or eating, and managing physical symptoms such as pain. At Millfield Healthcare, we focus on making sure whomever we care for feels as safe as possible. Having our compassionate, dedicated care assistants to talk to during a very isolating time can help. Our holistic approach places a strong emphasis on emotional support so that their quality of life is improved.
End-of-life care is the support given to clients who require peaceful and comfortable last weeks, months or years of their life. At Millfield Healthcare, we aim to enhance independence and provide support to improve our client’s quality of life and safety at this stage of life. We will help you (the person who is unwell, or the family) manage the emotional and physical conditions as well as possible.
